Required Tournament Documents For Each Team
Roster/Waiver forms will be emailed to each team 4 to 6 weeks prior to the tournament. For teams that wish to get a jump on completing your Roster/Waiver forms, here is a link to download the necessary form:

Please note that each players’ name and birthdate and his/her parent or guardian’s signature must be included on the form. Please also note that the team manager is required to sign the document in a number of places. Each team is required to submit a hard copy (no electronic copies) of this form at the Coaches’ meeting prior to the tournament or when picking up your team kits prior to your first game. If you do not submit your Roster/Waiver form, you will not get your kits and not be allowed to play.
We will send an email to whomever was listed on your Registration Form as the Team Administrator or Team Manager confirming the date, time and location of the Coaches’ meeting at least one month prior to the tournament. Additional details about the tournament will also be included in this communication.
You are not required to submit birth certificates. However, please have copies of the birth certificates of the players on your roster with you at all times. These documents will not be reviewed unless someone challenges the age of one of your players during the tournament.
